Reporting to an experienced Rehab Services Manager, the Physical Therapist will be supported by friendly nurses, skilled PT/OT/SLP colleagues, and an interdisciplinary care team including pharmacy, case management, and social work; as a Licensed Physical Therapist, responsibilities include comprehensive evaluations, individualized treatment planning, patient and family education, accurate EMR documentation, and collaboration on mobility, falls prevention, and discharge readiness, with mentorship, specialty consult access, manageable caseloads, and clinician-friendly workflows that keep a Physical Therapist focused on direct patient care.
