The Nurse Manager Case Management leads hospital-wide care coordination, utilization review, and discharge planning to optimize patient flow, reduce length of stay, and enhance outcomes. Daily responsibilities include rounding with interdisciplinary teams, reviewing caseloads, prioritizing barriers to discharge, coordinating authorizations and appeals with payers, facilitating care conferences, and collaborating closely with physicians, bedside nurses, social workers, therapy, and post-acute partners to ensure safe transitions of care. The Nurse Manager Case Management supervises and mentors a staff of 20+ case managers and social workers, manages staffing and scheduling, audits EMR documentation for accuracy and compliance, and ensures adherence to CMS, Joint Commission, and UR standards. The role monitors KPIs such as readmissions, avoidable days, and throughput, analyzes data dashboards, drives evidence-based workflows, and leads performance improvement initiatives and root cause analyses. Additional duties include standardizing processes, elevating patient experience, resolving complex placement issues, partnering with population health programs, and reporting outcomes to leadership.
What's Needed to Succeed:
- Degree from an accredited nursing program
- Licensed as a Registered Nurse RN in the state of New York or ability to obtain
- Progressive leadership experience overseeing care coordination, utilization review, or similar care setting
