The Manager Case Management will lead a collaborative department encompassing Care Management, Utilization Management, Care Coordination, and Discharge Planning. This organization is a 300-bed, full-service acute care hospital offering a Level III Trauma Center, Labor & Delivery, Level II NICU, and comprehensive medical and surgical services, providing clinicians with the opportunity to work in a diverse, high-acuity environment with exposure to a broad range of complex patient care. This clinical scope gives the RN Manager of Case Management an exciting platform to improve transitions of care, promote appropriate resource utilization, strengthen patient advocacy, and support safe, timely discharges.

Reporting to the Director of Case Management, the Manager of Care Coordination will receive support from experienced hospital leaders, friendly nurses, physicians, social workers, therapists, utilization review professionals, and a diverse team of healthcare specialists. Key responsibilities include guiding daily departmental operations, mentoring staff, coordinating complex cases, monitoring length of stay and utilization trends, supporting regulatory compliance, addressing barriers to discharge, and building effective partnerships across inpatient and post-acute settings 🏥. The Manager Case Management will also help cultivate accountability, professional development, open communication, and consistently patient-centered outcomes.
