The Manager of Case Management and Social Services will serve within a small community hospital that combines personalized care with access to essential acute-care and specialty services. Reporting to senior hospital leadership, the Care Coordination Manager will receive support from clinical executives, physician leaders, Quality, Revenue Cycle, Compliance, Risk Management, and community partners. Responsibilities include leading Case Management and Social Services staff, overseeing utilization review, resolving discharge barriers, monitoring patient status and medical necessity, and maintaining CMS and payer compliance. The Manager Case Management will also track length of stay, avoidable days, denials, readmissions, discharge delays, productivity, staffing, and departmental performance improvement goals.
