As part of an academic facility and a 150+ bed hospital, the Infection and Disease Pharmacist will collaborate on complex cases from critical care to med-surg and specialty service lines, leveraging pharmacist-driven protocols, pharmacist-to-physician consults, and data-supported stewardship dashboards that empower timely, targeted anti-infective therapy.

The Infection and Disease Pharmacist will provide prospective audit with feedback, optimize IV-to-PO conversions and renal/hepatic dosing, guide vancomycin and aminoglycoside PK/PD strategies, develop evidence-based order sets and guidelines, educate clinicians and trainees, and engage in P&T, DUE, and sepsis-improvement initiatives.
