Embedded on a collaborative team, the Licensed Clinical Social Worker will report to a Director of Behavioral Health within the hospital’s care management structure and partner closely with a Case Management leadership team. The Licensed Clinical Social Worker will conduct comprehensive psychosocial assessments, lead crisis intervention and short-term counseling, coordinate safe and timely discharges, support substance use and dual-diagnosis pathways, facilitate groups, and document within a modern EHR—while collaborating daily with friendly nurses, psychiatrists, hospitalists, therapists, pharmacists, and a diverse allied health team committed to whole-person care.
