Reporting to the RN Manager of Home Health and Hospice, the Registered Nurse Home Health will provide direct patient care to a selected group of Home Health patients. The Registered Nurse Home Health will supervise patient care personnel and delegate duties as required.
The Registered Nurse Home Health will identify and implement measures to prevent disease aggravation, ensure safety, make referrals to Rehabilitative Services, and coordinate other care. The Registered Nurse Home Health will convey empathy and compassion towards patients and their loved ones as they experience pain, grief, and illness.
