Registered Nurse Case Manager

An established acute care medical center located just outside of the Greater Boston region is currently searching for an experienced Registered Nurse Case Manager to join their award-winning healthcare team.

This acute care medical center is home to nearly 150-beds and provides a wide range of inpatient and outpatient medical and surgical services. This state-of-the-art medical facility is equipped to offer a variety of services including emergency and urgent care, primary care, perioperative services, oncology, pediatrics, and women’s health services. The Nurse Case Manager will work within a multidisciplinary group of patients, families, legal representatives, social workers, physicians, and internal teams. The Nurse Case Manager will be accountable for evaluating, organizing, and facilitating of patient progression through the continuum of care.  The Nurse Case Manager will achieve efficient, cost effective results through early assessment of pre-admission level of care, post-hospital discharge needs, review of available resources, and timely communication with the healthcare team and patients.

The Nurse Case Manager must have a minimum of 3-5 years of case management experience working in an inpatient acute care setting. The Nurse Case Manager must be able to think critically and utilize problem solving skills while maintaining safe and timely patient transitions of care. Prior knowledge of all insurance plan regulations including CMS/Medicaid to ensure compliance with all required processes and documentation.
