The Palliative Care Licensed Clinical Social Worker LCSW will join the family of dedicated nurses and other health care professionals living out a mission to heal people in body, mind and spirit. In collaboration with a multi-disciplinary care team, the Palliative Care Licensed Clinical Social Worker LCSW will be responsible for the timely coordination of health care services to meet the patient's individual need and promote quality and cost effective outcomes. The Palliative Care Licensed Clinical Social Worker LCSW will assist patients and families with psychosocial, emotional, financial or other barriers to care.

The Palliative Care Licensed Clinical Social Worker are trained to recognize the challenges in adjustment that often accompany the diagnosis of a serious illness like cancer and assist patients and families in managing distress. The Palliative Care Licensed Clinical Social Worker LCSW will facilitate connecting patients and families to community resources and in providing advocacy to those with cancer and their family/caregivers and friends.

The Palliative Care Licensed Clinical Social Worker LCSW will also offer multiple support groups for patients and families as well as crisis intervention and supportive counseling. The Palliative Care Licensed Clinical Social Worker LCSW in the survivorship role will help the patient cope with the uncertainty of recurrence.

The ideal Palliative Care Licensed Clinical Social Worker LCSW will have Master's degree in social work (MSW) from CSWE accredited university. LCSW required.  The Palliative Care Licensed Clinical Social Worker LCSW will have experience with psychosocial counseling in outpatient palliative care settings including survivorship programs required.  At least three years of inpatient, outpatient, or home based social work preferred.  Knowledge of CPS and APS mandatory reporting laws.
