Reporting to the Stroke Program Coordinator, the NP Stroke Program Nurse Navigator works in collaboration with Stroke Program key stakeholders to meet program certification requirements, including review of data and performance improvement projects. The NP Program Coordinator also works with Nurse Managers, Nurse Directors, Nursing Staff, and physicians, to ensure quality standards are met within the Neurology Department. The NP Stroke Program Coordinator will also have a Master’s Degree, State of CA Nurse Practitioner Licensure, and clean DEA.
Responsibilities for the NP Neurology Program Coordinator include providing stroke education, delivery and oversight of patient care, including post discharge follow up. This NP Program Coordinator also provides expert nursing consultation and practice oversight, and acts as a liaison between staff and patients’ families. Working with patients to ensure a good patient experience in providing excellent patient care is also a requirement.
