The Nurse Manager Home Health reports to the Chief Nursing Officer and is supported by the RN Clinical Supervisor. The Manager does daily rounds on the nursing units and monitors admissions to assess home care discharge needs. This person participates in discharge planning discussions with the patient, family, and healthcare team and serves as a liaison with Hospitals, Board and Care Homes, Skilled Nursing Facilities, Physician Offices, and Outpatient Discharge Planning Services.
The Manager Home Health will serve the Clinical Supervisor and Home Health RN team by empowering them to excel in clinical excellence, patient advocacy, and quality improvement. The Nurse Manager will get to be a change agent and transformative leader for the Home Health Department. They will also take oversight of budgetary maintenance, and will serve as a subject matter expert in Home Health Services throughout the facility.
