This reputable 250+ bed facility is well known for providing high quality care to the friendly community that surrounds them. They offer a number of services including but not limited to: Surgical Services, Maternal Child Health, Therapy and Rehabilitation, Diagnostic and Laboratory Services.
The Nurse Director Care Management will be responsible for managing the Case Management department including Social Care Services. The Director Care Management will be responsible for employee relations, fiscal planning, and initiating performance improvement projects within the department. The Care Management RN Nurse Director will be responsible for overseeing all Case Management functions including: social services, patient admissions, patient discharging and coordination of care plans.
The Case Management Director will support many different departments throughout this facility including but not limited to Women’s Services, Surgical Services, and the Cardiovascular Center. The Case Management Director will be supported by Nurse Supervisors, Nurse Case Managers and will report directly to the Chief Nursing Officer CNO and Vice President of Acute Care Services.
