Reporting directly to the Chief Nursing Officer (CNO), the Lead Social Worker Long Term Care will be responsible for the direction and implementation of the Social Services / Discharge Planning Department in the acute and assisted living facility associated with the hospital. The Social Worker will play an integral role in ensuring that patients and families alike are well educated and prepared for the next steps in recovery and rehabilitation.
The Lead Social Worker Long Term Care will provide orientation and education to the Social Services / Discharge Planning departments as well as to the multidisciplinary Nursing staff throughout the hospital. The Lead Social Worker will be responsible for developing, organizing and implementing programs, policies and procedures, conducts case work services and counseling and recommends any necessary changes to ensure the hospital and its patients continue to receive the best care possible. The Social Worker Long Term Care will oversee the performance improvement activities within the department and participates in Continuous Quality Improvement (CQI) activities in both the department and hospital as a whole. The Social Worker Long Term Care is a vital connection between patients and the fantastic services this hospital provides.
