The LCSW Manager of Care Management will report to the Director of Care Management and will provide oversight for the case management department. The Manager Social Work will be supported by a case management team of Social Workers, Nurses, Counselors, Therapists, and Department Assistants. This Manager will have the opportunity to work closely with Healthcare Leaders from across the Inpatient spectrum, and will serve as the key contact for external Agencies and Entities.
The LCSW Manager of Care Management identifies and resolves issues related to patient transitions and works collaboratively with other multidisciplinary care teams. The Social Work Manager will also take oversight of budgetary maintenance and serves as a subject matter expert in discharge planning, crisis intervention, and community resources.
