Reporting to the Nurse Manager of Home Health and Hospice, the Hospice RN Nurse Coordinator will oversee 12 FTEs including Hospice Staff Nurses and Home Health Aides. The Hospice RN Nurse Coordinator will draw upon their Hospice, Home Health, and/or Case Management leadership background to be an effective clinical resource to their team.
The Hospice RN Nurse Coordinator will oversee Hospice patient care and work compassionately and professionally with the patient, their family, and their care team. The Hospice RN Nurse Coordinator will consult with healthcare personnel at the hospital and related agencies to develop and carry out continuum of care plans.

Accredited by the Joint Commission, the hospital offers comprehensive services including inpatient and outpatient care, medical and surgical services, and diagnostic and emergency care.
