A full serviced acute-care hospital located in central Virginia is now searching for an experience Physical Therapist Case Manager of Home Health to join their respected healthcare team.
The Physical Therapist Case Manager of Home Health will be joining an accredited health care system that is comprised of multiple hospitals in throughout Virginia. This facility opened its doors almost 100 years ago with just under 50-beds and is now operating with roughly 100 licensed beds and proudly still providing care to the surrounding communities. This hospital is part of a much larger health system that has nearly 100 locations throughout the region that serves over half a million people throughout Central Virginia each year. This community-based acute care hospital is known to provide care to over 5,000 patient admissions and 25,000 Emergency Department visits each year.
The Physical Therapist Case Manager of Home Health will join a high-quality team of healthcare professionals that are proud to provide its patients with regional leading healthcare. The Physical Therapist Case Manager of Home Health will be solely responsible for the care coordination of all Physical Therapy patients that are being cared for by the organizations Home Health providers and will assume the supervision over the Nursing Assistants that are caring for their patients. The Physical Therapist Case Manager of Home Health will work with a variety of patients that require a diverse range of treatment including care for chronic illnesses, help after hospital stays, medication management, care for dementia patients, stroke recovery, other neurological rehabs (multiple sclerosis, ALS, Parkinson’s Disease, etc.), prevention of falls, and rehab recovery after joint replacements.
The Physical Therapist Case Manager of Home Health will be a pinnacle addition to the Home Health team and will work with a talented group of Home Health Nurses, PT’s and PT Assistants, Occupational Therapists and OT Assistants, Speech Pathologists, Medical Social Workers, and Home Health Aids. As the Physical Therapist Case Manager of Home Health, the ideal candidate will be responsible for managing the fiscal functions of the Physical Therapy procedures performed within Home Health and assure reimbursements are in coordination with CMS regulations.
