A Northern California non profit Medical Center is looking for a Home Health Physical Therapist to join their growing healthcare organization
The 250 bed hospital offers a comprehensive array of both inpatient and outpatient services to its surrounding community.
The Home Health Physical Therapist will provide rehabilitation care to those suffering from neck and back pain, neurological problems, and recovering from injury at their homes. The Physical Therapist will join a team of occupational, physical and speech therapists and will develop and evaluate treatment plans for patients referred by the practicing physician.
Treatment and recovery plans are formulated using a variety of methods and therapeutic practices to ensure restoration of body alignment, muscle strength, range of motion, coordination and pain management. The Physical Therapist will be seeing patients daily at their homes, and will design an individualized plan of care for each patient with measurable goals set for treatment. Additionally, the Home Health Physical Therapist will communicate via phone, email and video conference with patients.
The PT will perform physical therapy treatment in accordance to state and hospital guidelines and will perform necessary documentation of care, progress and will communicate with the Physician to create ongoing treatment plans.
