Reporting to the Chief Executive Officer, the Director Population and Community Health is responsible for Community Health activities for the market, aligning Population Health, Care Coordination, resources, and community partners. The Director Population and Community Health will collaborate with C-level leadership, Marketing Managers, Chaplains, Employee Wellness staff, and Volunteer Coordinators to meet strategic short-term and long-term goals.
The Director Population and Community Health oversees the Community Health Needs Assessment (CHNA) and Community Health Implementation Plan (CHIP). The Director Population and Community Health will ensure compliance with 501r regulations and implement best practices across the health system.
