This hospital is looking for a Case Manager who can meet each patient’s needs. This Care Manager will collaborate heavily with Physicians, nursing staff, other case managers, and the interdisciplinary team. This Care Manager will interview and asses each patient and family within 48 hours of admission. They will plan discharge, management family and patient expectations, and will identify the rate of readmission. This Case Manager will educate family members and the patient in order to arrange the needs and resources to reduce readmission rates. The Case Manager will be expected to participate in complex rounds, discharge planning huddles, and improvement team meetings. The Case Manager will follow the standards of this facility in order to maintain quality of care and recommend the best plan of action for each patient.
