This exciting opportunity will be targeted to Anticoagulation Clinical Pharmacist Specialist PGY2 candidates who have worked in the hospital inpatient acute care or medical group setting. The Anticoagulation Clinical Pharmacist Specialist PGY2 will be part of a non-profit and growing healthcare system that is dedicated to providing the best care possible for residents of the communities.

The Anticoagulation Clinical Pharmacist Specialist PGY2 will join a multi-specialty health care organization thrives to provide quality medical care, health education and research activities in a five-county area. The medical staff includes family practitioners, internists and pediatricians, as well as a range of specialist physicians. Services include imaging, nuclear medicine, a health library, health education programs, occupational medicine, diabetes education, nutrition counseling, infertility counseling, and weight management.

The Anticoagulation Clinical Pharmacist Specialist PGY2 will perform all duties necessary to provide daily pharmaceutical care to the patients of the Infusion Clinic, Cancer Treatment Center (CTC) and management of patients in the Anticoagulation. The Anticoagulation Clinical Pharmacist Specialist PGY2 will provide oversight for outpatient infusion medication order interpretation, entry, and compounding.

The Anticoagulation Clinical Pharmacist Specialist PGY2 will act as a drug information resource to patients, medical staff, nursing staff, and ancillary department personnel. The Anticoagulation Clinical Pharmacist Specialist PGY2 will compound and dispense prescribed medications and other pharmaceuticals for patient care by performing the related duties. This position reports to the Ambulatory Supervisor Pharmacist. 

The Anticoagulation Clinical Pharmacist Specialist PGY2 will also perform quality assurance, drug use evaluation, and adverse reactions reporting activities; participates in the application of all clinical services while accurately record and maintain information required by federal regulations, California laws, and department policy.
